Admissions to Cornerstone Gardens

Our Administrator, Admissions Coordinator, Director of Nursing, and Social Services Director coordinate admissions to Cornerstone Gardens, while our Quality Assurance team thoroughly reviews all referrals and inquiries.

Once we have received the necessary medical assessments and financial information, you can expect a prompt response on appropriateness for placement. Cornerstone Gardens is dually certified for Medicare and Medicaid programs, and we accept residents with third-party payments (insurance), Medicare and Medicaid (public) payments, and private funds. Our team will be able to assist you in determining your eligibility for insurance and/or medical payment programs, and we will help in completing all of the required forms.
