Social Services
The Social Services Department identifies residents' psychosocial needs, aids them in reaching their maximum potential, and facilitates discharge planning. Collaborating with all departments, the Social Services staff contributes to the development of a care plan, supporting residents in attaining their objectives, often involving a return to their home or a lower level of care after rehabilitation.
